Process:
• Registration is required.
• Prior to each CT appointment, patients will receive a phone call from our providers to determine eligibility.
• Once a CT scan is completed on the specified date, patients will receive their results via MyChart or a mailed letter.
• If there are abnormal results, patients will be contacted and follow-up appointment(s) will be made.

Current smoker between the ages of 50-77 with history of smoking one pack/day for 20 years or two packs/day for 10 years • Ex-smoker between the ages of 50-77 who has quit within the past 15 years.
